Wellness Screening Results

Are available for pick-up

**Supervisors: Thank you for posting this announcement in your department for employees who do not have ready access to email.

Lakeland Healthcare has delivered sealed envelopes containing the combined results of your March/April blood screening and wellness questionnaire. These are available for pick-up today, May 21 at the Human Resources employee benefits office on the second floor of the Administration Building

Please note that in order to meet HIPAA compliance, if you wish to pick up your spouse’s or your colleague’s results, written authorization via email to benefits@andrews.edu from your spouse or colleague respectively must be received before the results can be released to you.  If you prefer to submit a release form, you may pick one up at the employee benefits office.
